What a sad turn of events. I'm going to have to search for the article now as your link is broken. Just looking to see if there's any mention of a safety observer. That's an absolute must for any dive.
Ah. According to the article they did have a safety observer who sounded a Mayday when the pair didn't surface. However, they had no safety line attached so they lost their way in the cabin of a ship that had been sunk as an artificial reef.
