I've got a copy in my research library.   The book is laid out by state and then by town within the state.   
The states covered are : Arizona, California, Colorado, Idaho, Iowa, Kansas, Missouri, Montana, Nebraska, Nevada, New Mexico, Oklahoma, Oregon, South Dakota, Texas, Utah, Washington and Wyoming.   North Dakota must not have been very wild since it is not included.
