I need navigation related subjective data for my thesis, so I rigged a standard "Santa Barbara Sense-of-Direction Scale" to include some sensory information. I'd be very happy if you can also supply me with filled questionnaires of people with sensory impairments because such people are in fact quite rare in general population.

To facilitate participation I turned the standard SBSOD form into a web form that is very easy to follow, just click.

Done! There was no category beyond "Normal" to describe my hyperactive senses of smell and hearing, so that's what I chose. Aside from that, all other parts of your questionnaire went swimmingly.

Yes. The idea is to make use of the 20/20 vision acuity measure (or otherwise). Dioptre is a very objective measurement and people tend to remember what glasses they wear, while true acuity depends on many other things. To remain focused to the main idea of this questionnaire I had to make this compromise, as the 20/20 system is adopted mainly in US and not very consistently either. You'd be amazed how big differences are among various systems.

come to think of it my eyesight was perfect till they gave me wolfrin injections in my stomach back it 2001 when I had a heart attack
