That is a nice detail work. My wife has one that her dad made. First a small hole is drilled in the coin, then a very fine jewelers saw blade is inserted through the hole and worked around the coins design, by hand, cutting away the background. Then tiny files are used to clean it up. That was before moto tools, now I would use a tiny rotary cutter and a huge magnifier. You might be able to mask it off and use chemicals to etch it away. My hands and eyes are to far gone to try something like that anymore. Plus if patience were free I still would not buy any.
